The state of Texas offers a number of Community Care/Medicaid programs for which certain home care recipients may be eligible. As a Medicaid home health care agency, Nurses Unlimited is able to assist clients who receive in-home care under any of the various state programs:

Area Agencies on Aging (AAA)

Services Provided by Agency

In-home support with activities of daily living such as:

  • Cleaning
  • Dressing
  • Companion for Errands
  • Exercise
  • Feeding
  • Grooming
  • Hair/skin care
  • Laundry
  • Meal preparation
  • Medication reminders
  • Shopping
  • Toileting
  • Transfer/Ambulation

Eligibility Requirements

Services are provided to persons 60 years of age and older.

Community Attendant Services (CAS)

Eligibility Requirements

Financial eligibility is determined by the Texas Health and Human Services Commission, no age limit, have a functional limitation with at least one personal care task, based on medical condition, have a practitioner’s statement of medical need, have an unmet need for home management and personal care task(s).

Primary Home Care (PHC) Program

Financial eligibility is determined by the verification of the applicant’s Medicaid status by the Texas Department of Aging and Disability Services, be age 21 or older or be enrolled in the Consumer Attendant Services program, need help with at least one personal care task, have an unmet need for services, and have a practitioner’s statement of medical need.

Family Care (FC) Program

Eligibility Requirements

Financial eligibility is determined by the Texas Department of Aging and Disability Services, be age 18 or older, meet specified functional needs criteria, have unmet needs for home management and/or personal care tasks.

Medically Dependent Children Program (MDCP)

Eligibility Requirements

Financial eligibility for applicants not already receiving Medicaid coverage is determined by the Texas Health and Human Services Commission, be under age 21, meet the medical necessity criteria for nursing facility care, and meet disability criteria for Social Security or Medicaid benefits.

How to Apply for Services

Call 1-877-438-5658 to have the child’s or young adult’s name placed on the MDCP interest list.

STAR PLUS

Eligibility Requirements

Financial eligibility is determined by the Texas Health and Human Services Commission for applicants not already receiving Medicaid, be at risk of entering a nursing facility, be 21 or older, qualify medically and financially for Medicaid nursing home care, require services that can be adequately provided in the community within the assessed payment level, and choose Star Plus Services.
